Haircuts, clothes and wider lifestyle choices also feed into how each protagonist looks.
That pushes the series closer to the character simulation of Red Dead Redemption 2, while recalling the fitness and weight systems of GTA: San Andreas. Rockstar has not detailed stat meters, thresholds or specific combat penalties, so players should not assume a full survival-game layer. The confirmed player-facing effect is persistent visual change: neglecting sleep or fitness will not be a purely menu-level choice, and maintaining a preferred look may require planning around food, rest and activity.

The reactive-world element could matter even more during missions and free roaming. Carrying a rifle openly in public can trigger panic and police attention, making long guns a liability before a firefight begins. That creates a stronger reason to conceal weapons, pick an approach and consider where violence starts — rather than treating every street as consequence-free space. NPCs also vary by region, including Little Cuba and Mount Kalaga National Park, with different body types, fashion, tattoos, piercings and cosmetic surgery.

Jason and Lucia remain instantly switchable, including during a vehicle chase: Jason can drive while Lucia fires at pursuers after a quick swap. They can operate as a pair or separately, while their relationship changes in response to player decisions.
