How to Fish places its first serious progression gate on Island 2, the Forest: the Giant Piranha only appears after the lakeside Leech quest produces its unique bait. The clean route is Spider Crab shell hand-in, Island 2 access, three ground leeches, Leech Bait or Modified Leech, then a prepared Giant Piranha attempt. The key trap is treating this as a normal fishing objective. Leeches are environmental pickups, and the boss bait is single-use.

Bring exactly three leeches to the lakeside lady. Her active quest counter runs from 0/3 to 3/3; five is not the current requirement. Once she converts them, do not cast the bait until your weapon, ammo, healing, and dodge space are ready. A death or failed attempt costs the bait and sends you back to the leech search.

The Island Progression Chain

The Leech quest makes more sense when viewed as part of the game’s item-gated island route. Spider Crab is the earlier checkpoint, while Giant Piranha is the boss that provides the hand-in needed for Island 3.

Clear the Spider Crab Requirement First

You cannot reach the Forest and its Leech quest until the Spider Crab objective is complete. On the Starting Island, progress the fisherman’s Empty Beer Can interaction until the can functions as Spider Crab bait. Catch Spider Crab, keep its shell, and give the shell to the fisherman on the same island. That hand-in opens the route to Island 2.

This matters because a Spider Crab catch by itself does not finish the gate. The shell must be handed in before the next island becomes available. If the Forest remains inaccessible, check the fisherman’s quest state and make sure the shell did not remain in your inventory.

Activate the Leech Quest Before Picking Anything Up

On Island 2, find the young woman by the lake, identifiable by her blue overalls, and speak to her before searching. Her quest creates the visible 0/3 objective and establishes that the big fish she needs is attracted to leeches.

The activation order is important. Leeches collected before the conversation can fail to contribute to the quest counter, turning what should be one quick loop into another run through the Forest. Confirm that the counter is on-screen before leaving the lakeside area.

Find Ground Leeches, Not Fish

The quest leeches are small pickups on the Forest floor. They appear in grass and around vegetation or props near the lake; they are not a fish species you can reel in with the rod. Fishing the lake with standard bait will not advance the counter.

• Start at the lakeside lady and make one full sweep around the nearby lake and forest perimeter.
• Keep the camera low and angled toward the ground, where the small pickup prompts are easier to spot against the grass.
• Check around clusters of vegetation, props, and the outer edge of the walkable area.
• Watch the counter advance from 1/3 to 2/3 and finally 3/3.
• Return to the quest giver as soon as it reaches 3/3 instead of continuing to roam.

Leeches tend to spawn near one another. Finding one is a reason to inspect the surrounding patch of grass carefully before resuming the perimeter sweep. There are enough on Island 2 to finish the objective in a single pass when the quest is active.

Turn Three Leeches Into the Only Valid Boss Bait

Hand all three leeches to the lakeside lady. She converts them into one boss-only item, shown as Leech Bait or Modified Leech. Both labels refer to the bait needed for Giant Piranha.

No store lure, generic bait, or money grind substitutes for it. Giant Piranha hooks only on this quest reward, so buying more ordinary supplies does not bypass the Leech gate.

Warning: Leech Bait is consumed when it is used. If the cast fails, the catch is lost, or the fight ends in death, the bait does not return. You must reactivate the practical loop by gathering three more ground leeches and completing another turn-in.

Prepare the Arena and Loadout Before You Cast

Giant Piranha is a 100-health boss-fish, but the danger comes from the opening swarm of smaller piranhas and the way the boss compresses your movement. Treat the Leech Bait as an attempt token, not as an item to test with an underprepared setup.

• Equip a gun: the basic pistol is the minimum practical choice. A strong shotgun is better when available because higher close-range firepower shortens the time spent under swarm pressure.
• Top up ammunition: running dry after the small piranhas arrive leaves the boss fight needlessly exposed.
• Carry healing: have recovery items ready before baiting, since bites from the boss and adds can stack quickly.
• Choose open ground: identify a wide shoreline or clear space with room to strafe left and right.
• Remove nearby distractions: clear minor threats and avoid hazardous areas before starting the encounter.

Open lateral space is more valuable than a cramped position near cover. Giant Piranha’s jump-and-bite pressure is easier to avoid by moving sideways. Backing into the waterline, walls, dense props, or other obstacles cuts off the lane you need to survive.

Avoid Pufferfish poison while setting up. Its purple poison damage ticks become dangerous fast, and starting the Piranha encounter while already losing health wastes the healing margin needed for the first swarm.

Hook Giant Piranha and Control the Swarm

With the area prepared, equip the Fishing Rod, select Leech Bait or Modified Leech as the active bait, and cast into the water near the Island 2 quest area. Reel steadily until Giant Piranha takes the bait. That hook starts the second major boss encounter.

The fight opens with smaller piranhas. Do not spend the entire attempt trying to erase every add. Clear enough of the swarm to create a safe firing lane, then keep sustained damage on Giant Piranha. The boss is the objective, and its 100 health is the health bar that ends the encounter.

• Move laterally through the clear ground chosen before the cast.
• Use short, controlled repositioning instead of retreating blindly toward the water or obstacles.
• Thin the small piranhas when they block your line of fire or threaten to surround you.
• Return damage focus to Giant Piranha as soon as the lane is open.
• Use healing early enough to survive the next bite sequence rather than waiting for a last-second recovery.

The pistol can complete the fight, but the stronger shotgun route has a practical advantage: it reduces the amount of time the swarm has to turn the arena into a damage race. Keep moving even while using higher firepower. Standing still to force damage is the usual way a manageable attempt becomes another leech farm.

Keep the Quest Drop and Unlock Island 3

After Giant Piranha falls, keep the special progression drop. Its name can appear as a Giant Piranha Skeleton, a tail, or a special big yummy fish item, depending on the build or item text. It is distinct from ordinary meat and should go straight back to the lakeside lady.

Giant Piranha is worth $11,000 as a catch, a major economic reward at this point in the run. The quest hand-in matters more for progression: giving the special drop to the lakeside lady awards the coordinates required to continue to Island 3. Do not sell or discard the unique drop before that hand-in.

Recovery Checklist After a Failed Attempt

A failed Giant Piranha attempt resets the practical gate because the bait is gone. Rebuild the attempt in the same order: return to the lakeside lady, confirm the Leech quest counter, sweep the nearby ground for three pickups, convert them into fresh Leech Bait, then restock before casting again.

• Do not search the water for replacement leeches.
• Do not collect a fifth leech; the correct target remains 3/3.
• Do not cast fresh bait before checking gun, ammunition, healing, and clear lateral space.
• Do not sell the special Giant Piranha drop after a successful kill.
