No Rest for the Wicked Focus Regen Guide: Gems and Enchantments

Treat Focus Regen, Focus Gain, and maximum Focus reductions as separate modifier types. A Focus Regen line is a regeneration effect; Focus Gain is a scaling stat; and a reduced maximum-Focus value is a capacity downside that does not create, block, or explain regeneration by itself.

The supplied official notes do not publish a numeric build identifier for a universal “current version,” so use those dated PC changes as the reliable mechanics reference rather than applying older Beta behaviour or an assumed combat-only rule.

If your setup looks like it has “no passive Focus regen,” the first thing to do is not panic-scrap the item. Read the exact wording on the gem or enchantment. In No Rest for the Wicked, similar-looking Focus lines can point to very different systems: passive recovery over time, scaling on Focus earned from other actions, a condition that must be active first, or a simple change to the size of your Focus pool.

That table is the whole argument in miniature. If the line says regenerate, treat it as regeneration. If it says gain, treat it as gain. If it changes maximum Focus, treat it as capacity. And if it references a state like Full Focus or Low Health, treat it as conditional until proven otherwise.

A quick mechanic map before you blame the gem

A clean way to sort a confusing item is to ask one question: When should this effect pay out?

  • Passive refill over time: wording such as Focus Regen or Regenerate Focus.
  • Attack-based or base gain scaling: wording such as Focus Gain.
  • Trigger-based refund: wording tied to a hit, kill, damage event, or another specific trigger.
  • Bigger or smaller Focus pool: wording that changes maximum Focus or Focus capacity.

Those are not interchangeable buckets. Patch 1 explicitly connects regeneration values and Focus Gain scaling, but it does not collapse every Focus-related modifier into one universal rule.

Why a Focus Effect Can Look Inactive

The most important Patch 1 wording is specific: “Regenerate Focus” enchantments scale with Focus Gain. That means an item whose effect explicitly says it regenerates Focus can have its value affected by a separate Focus Gain stat. It does not establish that every modifier mentioning Focus regenerates it over time.

This is where a lot of player confusion sneaks in. A stat can matter to a regeneration effect without being regeneration itself. So if you equip something with Focus Gain and expect a new passive tick to suddenly appear, you may be looking for the wrong behaviour. The stat may be relevant, but only because it scales an effect that already says Regenerate Focus.

Check the tooltip for a condition before treating the effect as broken. A gem tied to Full Focus, Low Health, or Full Health only applies while its listed state is true. The Together Update also changed where many of these conditional effects live: they were moved from enchantments to Gems, and Gems were given set values rather than roll ranges. A newly found gem can therefore behave differently from an older enchantment with similar-looking Focus language.

A maximum-Focus penalty needs to be evaluated separately. It reduces the amount of Focus available in the pool; it does not convert regeneration into Focus Gain, and it does not supply evidence that regen only works during combat. Keep the downside in mind when judging an item’s value, but do not use it as the explanation for a missing regeneration effect unless the tooltip directly connects the two.

Worked example: two Focus lines that are not doing the same job

Use a placeholder example to keep the logic straight.

Say you have a helm with the line “Regenerate Focus +X”. Read that as passive recovery over time. Patch 1 supports that category directly, because the official notes say Health and Focus regeneration values on enchantments and gems were doubled.

Now compare it with a weapon or accessory that says “Focus Gain +Y%”. Read that as a scaling stat, not as proof of passive regen. It can matter to a regeneration enchantment because Patch 1 says Regenerate Focus enchantments now also scale with Focus Gain stat, but the tooltip still is not the same thing as a raw regen line.

Then add a gem with “Maximum Focus -Z” as a downside. That changes how large your Focus pool is. It does not, by itself, tell you anything about whether the helm’s regeneration is valid or whether the weapon’s Focus Gain is working.

And if another gem says “at Full Health” or “at Full Focus”, that is a separate gate again. If the state is not active, the effect can look dead even though the item is behaving exactly as written.

Do Not Use the Old 110% Focus Rule

Older Beta discussion described Focus regeneration beginning at 110% Focus. That historical behaviour is not a current rule to build around. The later official patch notes changed Focus regeneration values, added Focus Gain scaling to Regenerate Focus enchantments, and reworked where many conditional modifiers appear.

That is the cleanest way to reconcile the mixed community claims. Older behaviour existed in discussion, but the safer read for current PC mechanics is the official dated patch baseline: Patch 1 for the regen and scaling changes, then the Together Update for the relocation of many condition-based effects to Gems. When those two updates changed both the numbers and the placement of item effects, old shorthand stopped being a reliable decoder ring.

There is also no official patch-note rule stating that Focus regeneration from gems or enchantments is universally disabled outside combat. Avoid removing or replacing an otherwise useful item on that assumption. The actionable check is the item’s exact wording: regeneration effect, Focus Gain scaling, a listed condition, or a maximum-Focus downside.

A non-destructive troubleshooting flow for Focus gems and enchantments

If you want a fast sanity check without rebuilding your character, use this order:

  1. Identify the wording. Is the line Regen, Gain, Max Focus, or a condition?
  2. Check whether the item is explicit. “Regenerate Focus” is stronger evidence for passive recovery than a generic Focus-related line.
  3. Look for a state requirement. Full Focus, Low Health, and Full Health are not decoration. They are the switch.
  4. Separate capacity from recovery. A smaller maximum Focus pool can make the bar look odd, but it does not explain away a missing regen effect by itself.
  5. Re-check any Focus Gain pieces in the setup. Patch 1 made Regenerate Focus enchantments scale with Focus Gain, so Gain can matter even when it is not the source of passive recovery.
  6. Ignore Beta-era rules unless the current tooltip still supports them. The dated PC patch notes are the better baseline.

That flow will not answer every edge case, because the public notes do not publish a rulebook for every single item line. It does, however, keep you from mixing up four different mechanics and blaming the wrong one.

Safe Checklist for a “Focus Not Regenerating” Setup

  • Confirm whether the effect says Focus Regen/Regenerate Focus or Focus Gain.
  • Read every condition on the gem or enchantment, especially Full Focus, Low Health, and Full Health.
  • Separate a reduced maximum-Focus value from the item’s recovery effect; it changes capacity rather than defining regeneration.
  • For an explicit Regenerate Focus enchantment, account for Focus Gain because Patch 1 made that stat part of its scaling.
  • Use the June 25, 2025 Patch 1 notes and January 22, 2026 Together Update notes for PC interpretation; do not apply unverified console behaviour or older Beta thresholds.

If you keep those five checks in front of you, most “this gem does nothing” cases become much easier to sort. The trick is not discovering a hidden universal rule. The trick is refusing to mash regen, gain, condition, and capacity into the same box.
