Why the Care Center Is So Rough (and How This Guide Helps)

After spending a solid evening trapped in Resident Evil Requiem’s Care Center, I finally pieced together a clean route that grabs every quartz, key item, and antique coin without wasting ammo or dying to Chunk for the twentieth time. The area is a classic Resident Evil knot: West Wing, East Wing, multiple keycards and ID wristbands, stealth-only sections, and three linked Moon, Sun, and Star quartz puzzles that all gate a single “escape” door.

This walkthrough takes you step by step from your first West Wing Keycard all the way to opening the Moon-Sun-Star door, including:

  • Exact safe combinations and puzzle inputs for the Moon and Sun quartz
  • Where to find the Hemolytic Injector and how to use it for stealth kills
  • All key antique coin locations on the critical path
  • When to use your ultra-rare Requiem ammo (and when to save it)
  • How to deal with the butcher and Chunk without burning through resources
  • How to get the Moon, Sun, and Star quartz and what happens after you place them

If you play methodically, expect about 60-90 minutes for your first full clear of the Care Center. Once you know the route below, you can cut that almost in half.

Step 1 – Get the West Wing Keycard and Your First Safe Room

From the starting corridor of this section, head into the room on the right. This is your first safe room in the Care Center:

  • Use the typewriter to save.
  • Interact with the storage box and deposit any item to pop an early achievement/trophy.
  • Grab the Green Herb on the shelf.

Go through the door with the red light above it to trigger a cutscene. When it ends, you’ll automatically receive the West Wing Keycard.

Enter the West Wing

Backtrack to the locked West Wing door and swipe the keycard. Inside, go left down the main hall. On your right you’ll see a heavy door with a circular slot – this is where the Moon, Sun, and Star quartz will eventually be placed. For now, ignore it and continue through the door on your right to meet your first major threat.

Stealth Past the Butcher (Do NOT Waste Your Requiem Yet)

Inside, the hulking butcher (chef) enemy is busy carving up something nasty. On my first run I tried to fight him with the Requiem pistol’s single bullet. It was a mistake; it barely slows him and leaves you underpowered later.

  • Crouch immediately and stay low.
  • Wait for the butcher to walk to the far side and start working at the cart.
  • While he’s focused, quietly push the cart forward until he barks “Next!” or a similar line.
  • As soon as he moves again, back off and let him re-engage with his work.
  • Use his distraction window to finish moving the cart and slip through the left-hand door.

Alternative (riskier): You can kite him back towards the entrance, shoot his leg, and finish with a melee hit to stun him long enough to escape. I only recommend this if you’re very comfortable with his attack timing.

Step 2 – Arm Up in the West Wing and Get the Moon Quartz

First Infected, Handgun, and Knives

Beyond the butcher room, you’ll encounter your first standard infected. For now, run past him and go right to unlock the shortcut door back to the save room. Use it to save if you like, then return and this time take the right-side corridor fully.

  • Room on the right: empty handgun (no ammo).
  • Door ahead and then left: find a makeshift knife and a few handgun rounds.

With ammo and a blade, go back to the first infected:

  • Shoot twice to stagger.
  • Hit the melee prompt for a heavy follow-up.
  • Finish with the knife to conserve ammo.

The infected drops a sturdier Hunting Knife. Grab it, then head left along the hall to pick up an Antique Coin. Smash the nearby crate for extra ammo, then be ready: another infected shambles down the stairs. Deal with it, then interact with the valve wheel – it’ll snap off, forcing you to find another way later.

Bar & Lounge – Safe Combo and Coins

Go up the stairs and grab the West Wing map from the wall. Continue to the top for a Green Herb and more ammo, then drop back down and turn left into the Bar & Lounge.

In here, you can optionally fight three infected. It’s resource-heavy, but worthwhile if you want all collectibles:

  • Kill the woman in the white dress to earn an Antique Coin.
  • On the piano: another Antique Coin.
  • Behind the bar is a safe containing three Antique Coins.

Safe combination (bar safe): Left 10, Right 80, Left 30. Remember the direction changes – I lost a couple of minutes spinning it wrong the first time.

There’s also a second handgun and more ammo if you fully clear the room. Once you’re stocked, exit through the right-hand door and continue straight into the office area.

Chairman’s Office – Unicorn Box and Moon Quartz Puzzle

In the small office before the Chairman’s room, check the white coat for a note hinting at the upcoming puzzle. Then enter the Chairman’s Office itself.

  • On the left: the Unicorn Trinket Box. Rotate it to find a hidden switch on the back – this reveals a Red Jewel.
  • Scattered around: a pencil, ammo, and a note that foreshadows the Moon–Sun–Star sequence.

Back in the previous room, you’ll see the first puzzle box on the table. This one contains the Moon Quartz. If you want the solution straight up, here it is:

Moon Quartz puzzle input: Moon → Sun → Star → Moon

Press the symbols in that order and the box pops open so you can claim the Moon Quartz. Return to the main West Wing corridor and slot the Moon Quartz into its place on the big Moon–Sun–Star door frame.

Red Jewel Door and East Wing Keycard

Remember that strange door at the start of the West Wing that needed something inserted? That’s for the Red Jewel from the Unicorn box.

  • Backtrack to that door and use the Red Jewel as the key.
  • Inside is a “parlor” area with a vending machine you can feed Antique Coins into for upgrades and items (hip pouch, stabilizer, healing boosts, Blood Collector upgrade, etc.).
  • Grab the Antique Coin on the ground near the entrance.
  • On the right, high on a shelf: another Antique Coin.
  • On the northwest side, smash the vase for ammo, then pick up yet another coin just to its left.

On a severed hand at the north end of the room is the East Wing Keycard. Take it – this is your ticket to the next half of the Care Center.

Step 3 – Enter the East Wing, Hemolytic Injector & Blood Lab

Entrance, Mr. Raccoon, and East Wing Safe

Use the East Wing Keycard at the corresponding door. As soon as you step inside, look for the Mr. Raccoon figure on the counter – shoot it for the series’ traditional collectible.

To its right is a locked drawer that requires a small key. We’ll come back after we get the lockpick.

Grab the Green Herb to the left, then go into the room on the right. You’ll see:

  • A prone corpse in the center (you’ll revisit it later).
  • Two infected shambling around.
  • A safe on the right wall.

Safe combo (East Wing ward safe): 30, 10, 50. This one doesn’t depend on direction – just dial the numbers in order. Inside is more ammo and supplies.

Hemolytic Injector – Free Stealth Kill (and Trophy)

Crouch and carefully move along the right side of the room. Near the closer infected you’ll see the Hemolytic Injector on the floor or table.

  • Pick it up while staying crouched.
  • Approach the nearest infected from behind until you see the stealth prompt.
  • Use the Hemolytic Injector for an instant kill – this also unlocks a stealth-kill achievement/trophy if you haven’t done it before.
  • Deal with the second infected using your handgun and knife.

Exit via the door on the right side of where you performed the stealth kill and turn on the light switch in the corridor – trust me, you don’t want to be fumbling here in the dark when Chunk starts roaming later.

Analyze the Blood Specimen and Unlock Crafting

In the next room, open the first locker for Scrap (crafting material). On the right by the sink is the Blood Collector. Pick it up and you’ll automatically be tasked with analyzing a blood specimen.

  • Grab the Denatured Blood Specimen from the nearby counter.
  • Use the analysis machine right next to it.
  • Once the analysis finishes, new recipes unlock in your crafting menu – including handgun ammo and additional Hemolytic Injectors.

This is your lifeline for the rest of the Care Center. From here on, always suck up blood from downed infected when it’s safe – you’ll need it for ammo and injectors.

Step 4 – Lead Researcher’s Office, Level 1 Wristband & Sun Quartz

Leave the blood lab and take the first left in the corridor. You’ll pass the locked Isolation Ward on your left (ignore it for now; you don’t have the security clearance). Keep heading north, dodging or dispatching enemies as you go, then duck into the first door on your right to unlock a handy shortcut.

Get the Lockpick and Level 1 ID Wristband (Meet Chunk)

Go back out, take a right, and head up the stairs. Two zombies wait at the top. My trick is to lure them halfway down the stairs, then sprint past them back up while they’re still turning.

  • Enter the Lead Researcher’s Office.
  • On the desk: a lockpick. This opens the drawer back at the East Wing entrance for a rare material (worth grabbing for crafting).
  • Examine everything – especially the locked, Corrosive-protected padlock box you can’t yet open. This box holds the Sun Quartz.

Leave the office and head left down the hallway. You’ll find the Level 1 ID Wristband on a table by a security door. The moment you take it, a cutscene introduces Chunk, the massive pursuer monster for this area.

Important: At this point, do not try to kill Chunk. I wasted multiple Hemolytics and a pile of ammo learning he’s effectively unkillable until later. Just run and use the newly opened doors that your wristband can access.

Find Corrosive and Open the Sun Quartz Box

Once you shake Chunk by weaving through rooms and using corners (listening for his heavy footsteps helps), head to the room north of the Lead Researcher’s Office that your Level 1 wristband now opens.

  • Inside are several zombies – clear them carefully, using the environment to funnel them.
  • On a shelf: the crucial Corrosive item.

Return to the Lead Researcher’s Office. A cutscene with Victor plays as you approach the padlocked box. After it ends, use the Corrosive on the lock to melt it away and reveal the second puzzle box.

Sun Quartz puzzle input: Star → Sun → Moon → Sun

Enter that sequence to obtain the Sun Quartz. Next to the box is a leather notebook – this contains the instructions for unlocking an Organ Transport Box you’ll find later in the garage. Read it or at least note the pattern; you’ll need it soon.

Step 5 – Pantry, Wrench, Organ Parts and Level 2 Wristband

Ward Office, Requiem Bullet and Optional Butcher Kill

Use your Level 1 Wristband to open the Ward Office (back through the shortcut room you opened earlier). Inside you’ll find:

  • Converged Blood Specimen – analyze it in the blood lab to unlock more advanced crafting options (including Requiem ammo).
  • Extra handgun ammo and scrap.
  • A briefcase containing a Requiem round.

Now is a good time to go back to the West Wing and reassess the butcher. You can continue to sneak past him forever, but if you want the Pantry Key and its loot:

  • Make sure you’ve crafted at least one extra Requiem bullet or bought one from the parlor with coins.
  • Face the butcher in his kitchen arena.
  • Land two clean Requiem shots to the head or chest – this will drop him.
  • Loot his body for the Pantry Key.

Unlock the pantry door off the kitchen to grab extra ammo and supplies. This isn’t strictly required for progression, but the resources help a lot once Chunk starts roaming both wings.

Custodian’s Office, Organ Transport Box, and Bulldozer Trap

From the kitchen side, exit through the other door and use your wristband on the next locked door to enter the Custodian’s Office:

  • Use the storage box and save point here – this becomes your West Wing hub.
  • Loot everything, including the Wrench, which replaces the broken valve wheel.

Go through the blue door out of the Custodian’s Office into the garage. Move forward to find the Organ Transport Box. This is the one referenced in the leather notebook.

Interact with it and input the sequence from the notebook (it’s always the same pattern the note shows). The box opens to reveal the Artificial Lungs – one half of what you’ll need for the corpse in the East Wing.

As you proceed deeper into the garage, a zombie crashes a bulldozer through the wall and starts pushing towards you.

  • Shoot the zombie driver – the first shot breaks the glass, the second kills him and stops the dozer.
  • Crouch and quickly crawl under the halted machine before Chunk or other infected catch up.
  • Grab the loot on the right, then exit the garage back towards Cold Storage.

Cold Storage, Artificial Heart, and Reviving the Corpse

Use the Wrench on the Cold Storage door to open it. Inside, among the chilly cadavers, you’ll find the Artificial Heart on a tray.

Now you have both Artificial Lungs (from the organ box) and Artificial Heart. Head back to the East Wing room where you first saw the body laid out near the safe.

  • Interact with the corpse to insert the lungs.
  • Interact again to insert the heart.
  • After a short, very creepy animation, the corpse “revives” and presents you with the Level 2 ID Wristband.

This upgrade is huge: it opens the Isolation Ward and, more importantly, the path to the Security Manager’s Office where you’ll find the final Star Quartz.

Step 6 – Star Quartz, the Blind Girl, and the Fake Escape

Reach the Security Manager’s Office

With Level 2 clearance, loop back past the Isolation Ward and unlock it. Inside are more enemies and some extra blood specimens and supplies – clear it if you’re low on crafting materials.

Use the new doors marked for Level 2 clearance to push deeper into the East Wing until you reach the Security Manager’s Office. A story cutscene plays here, introducing a blind girl held captive behind security glass and bringing Chunk back into the mix as a roaming threat.

Use the Blind Girl to Solve the Braille Star Puzzle

In the Security Manager’s Office you’ll find the third puzzle box on the desk, along with a note written entirely in Braille. The trick, which took me way too long to realize, is that you can’t read it – but the blind girl can.

  • Interact with the intercom to talk to her.
  • Examine the Braille note while she’s listening; she’ll “read” it aloud as a sequence of symbols or taps.
  • Match that sequence on the puzzle box buttons – just press them in the exact order she recites.
  • If you mess up, you can repeat the process as many times as needed.

When you input her sequence correctly, the box opens and you finally obtain the Star Quartz. A cutscene here also upgrades your wristband to Level 3, letting you free the blind girl and unlocking a few extra doors if you want more loot before you leave.

Place All Three Quartz and Open the Moon–Sun–Star Door

Now you’re ready for the “escape.” Backtrack (carefully – Chunk now patrols more aggressively) to the big door frame in the main West/East connecting corridor.

  • Place the Moon Quartz (from Chairman’s Office) in the left slot.
  • Place the Sun Quartz (from Lead Researcher’s Office) in the center slot.
  • Place the Star Quartz (from Security Manager’s Office) in the right slot.

The mechanism grinds to life and the massive door unlocks. This feels like your exit from the Care Center – and mechanically, it is – but story-wise, Requiem pulls the rug out.

Major story spoiler warning for the end of this section:

Past the door is a short final stretch – a hallway and staging area that funnels you into a cutscene. You think you’re escaping Rhodes Hill’s Care Center, but instead you’re captured and dragged deeper into the facility. The game switches gears (and, later, playable characters), revealing that the Care Center was only the first layer of a much bigger nightmare. You survive the area, but you absolutely do not escape.

Wrap-Up – What You’ve Gained from the Care Center

By following this route, you leave the Care Center section with:

  • All three quartz (Moon, Sun, Star) successfully collected and used.
  • Most, if not all, Antique Coins in the area and parlor upgrades bought.
  • The Blood Collector and multiple analyzed specimens, unlocking crucial crafting recipes.
  • The Hemolytic Injector and its stealth-kill trophy checked off.
  • Requiem ammo pickups and a clear sense of when to spend them (butcher, later Chunk) instead of wasting them on regular infected.
  • Level 2 and eventually Level 3 ID Wristbands and all the shortcuts they open.

The next area builds directly on the resources and knowledge you’ve picked up here. If you managed to get through the Care Center with a few Requiem shots, spare Hemolytics, and a healthy blood stockpile, you’re in a great position for what comes next. And if you stumbled your way through like I did on my first attempt, don’t worry – once you internalize this route, the whole section becomes a tense but manageable sprint instead of a brick wall.
