For the established cross-save workflow, both the old and newer versions need to be updated to version 4.0. Load the latest save in the older version, create a fresh save, upload it to the cloud, then launch the upgraded version and download it. Do that before patch day if the save matters. Ten years of patches have taught everyone the same lesson: “it should be fine” is not a backup strategy.
The current Remastered guidance says earned achievements and trophies transfer with the upgrade. That matters most on PlayStation, where the original PS4-to-PS5 next-gen transition had the distinctly irritating reputation of preserving the save while leaving players to earn the trophy list again.

CD Projekt Red’s newer clarification changes that expectation for Remastered: completed trophy and achievement requirements are meant to carry forward alongside the save. Players with a finished Skellige deck, a completed contract log, or an old Death March clear should not need to manufacture another 100-hour save file solely to make their profile look complete.
There is still a sensible precaution here. Sync the console profile and cloud save before applying the update, then check the trophy or achievement list after launching the upgraded client. The stated policy is transfer; a platform storefront failing to refresh its records is the sort of small administrative mess that can turn a clean upgrade into an evening of support tickets.

Progress carries over, but skill allocations and settings do not. Remastered resets both, allowing players to immediately reallocate their ability points after booting the game. Inventory, quest progress, character level, and the broader state of the save stay intact; the game simply refuses to preserve the exact arrangement of combat, Signs, alchemy, and general skills built under the previous client.
It is a minor inconvenience for a straightforward build and a larger one for anyone who tuned mutagens, decoctions, enchantments, and ability slots around a particular playstyle. Take screenshots of the character screen and gameplay settings before updating. The upgrade offers an instant respec in practice, but it cannot remember why you put those points where you did.

Remastered also carries CD Projekt Red’s “one more patch” promise: cross-platform mod support through mod.io for PC, PS5, and Xbox Series X|S. Console players access approved mods from an in-game menu rather than downloading files manually. It is the same compromise used whenever a publisher wants mod support without handing a locked-down console filesystem the keys to Kaer Morhen.
Using that feature requires a mod.io account linked to a CD Projekt Red account. Not every PC mod will be eligible for consoles, since each approved mod has to meet technical and moderation requirements for the platform pipeline. This is curated console modding, not a promise that every wild script overhaul from the PC scene will appear on Xbox or PlayStation.
Nintendo Switch 2 is part of the Remastered release plan, but CD Projekt Red has not detailed the exact console-mod workflow or limitations for that version. Keep expectations parked until the studio gives the platform-specific answer.
