Mandrake Ward is unlocked through Lacra’s The Night of Horrors quest, and the required choice is to eat the mandrake in its final cave sequence. Mandrake Ward Requirements
Mandrake Ward sits in the Vampirism skill tree, but reaching the tree does not make it available. The game locks the perk behind Lacra’s quest chain and the decision made during The Night of Horrors. Treat the following route as mandatory:
- A Friend Like This
- Song of the Mountain
- Hive and Seek
- Our Rotten Roots
- The Night of Horrors / Misery Loves Company
You do not need to complete Lacra’s eventual ending quest, Midnight Reckoning, to gain Mandrake Ward. The relevant cut-off is the mandrake decision within The Night of Horrors.[4]
Important: do not spend time checking vendors, Witchcraft manuals, or unrelated points of interest for this perk. Mandrake Ward has no normal purchase or exploration route. A vial of Lacra’s blood can provide an alternative route to the Mesmerize ability in some circumstances, but it does not replace the Mandrake Ward unlock.[3][9][14]
Step 1: Finish Our Rotten Roots and Trigger The Night of Horrors
Complete Our Rotten Roots before returning to Westridge Outpost Tower. The quest start timing is presented slightly differently across current walkthroughs: one route begins after waiting one in-game day, while another instructs you to wait a couple of days. The practical sequence is simple: pass one day, return to Lacra at night, and select the research conversation. If The Night of Horrors is still absent, advance another day and return at night again.[2][4]
The quest can also be offered around the Brencis encounter if you speak with Lacra before that fight, but Our Rotten Roots remains the key progression requirement. Use the dialogue at Westridge Outpost Tower rather than searching the map for a separate quest marker.[1][4]
If Coen needs a stronger baseline before committing to Lacra’s night quest, use the early-build guide for broader Vampirism, Swordmastery, and Witchcraft planning. Mandrake Ward is a survival tool, so it works best when the rest of the build can still clear mandatory enemy groups efficiently.

Step 2: Follow the Quest Route Without Burning the Night
The Night of Horrors is a substantial night commitment. The full route uses roughly six to seven time segments, with the longer result tied to optional relationship content. Avoid treating it as a free-roam excursion: unnecessary detours and fights can consume enough time for the night to end before the mandrake sequence.[2][4]
After speaking with Lacra, uncover Kallias’s secrets and reach the roadside shrine. From there, the will-o’-the-wisps are the route markers that matter. Follow them through the swamp instead of navigating by distant map landmarks. They lead you from the early investigation points toward the deeper sections of the mission.[2][4]
- Reach the roadside shrine after the initial Kallias investigation.
- Follow the wisps through the swamp.
- Inspect the stone slab, then follow the wisps again.
- Defeat the required lizard enemies and investigate the marked area.
- Clear the mandatory hellhounds and inspect the rockslide.
- Enter the sunken crypt, defeat its undead, and inspect the sarcophagus.
- Defeat the spirits, then proceed into the hidden cave with Lacra.
The enemy groups on this route are part of the mission structure. Optional encounters away from the wisps are the ones to leave alone. Preserve healing and time for the crypt, the spirit encounter, and the final cave approach; those are the sections that advance the quest.[2][4]
Players still working through the opening chapters can use the Vale Sangora prologue route to protect early time segments before moving into companion questlines. That route covers early-game scheduling; Mandrake Ward itself begins only with Lacra at Westridge Outpost Tower.
Step 3: Eat the Mandrake in the Hidden Cave
Spoilers for Lacra’s questline and relationship path follow.
In the hidden cave, check in with Lacra after the spirit fight. She presents the mandrake and gives Coen the decisive choice. Select Eat the Mandrake. This causes Coen and Lacra to share its power; there is no version of the scene where only one of them consumes it.[4][6]
Refusing the mandrake ends this part of the night without Mandrake Ward. There is no later vendor, respec option, ordinary skill unlock, or alternate quest route that restores access to the perk during that playthrough.[3][4][9]
Eating the mandrake also opens the Lacra romance path, although romance is optional for Mandrake Ward. The sequence can proceed into explicit content if you choose the later relationship dialogue, including the “Kiss Her” option. Players seeking the perk while avoiding that material can eat the mandrake, gain the unlock, and decline the later romance progression.[6][14]

Step 4: Check the Vampirism Tree at a Shrine
After eating the mandrake and completing the quest, visit a shrine and open the Vampirism skill tree. Mandrake Ward should be available there. Current guide documentation differs on the final cost: some versions describe the perk as automatically granted by the mandrake scene, while other current routes require one perk point at a shrine.[1][2][4][9]
The safe approach is to inspect the perk before assigning any point. If Mandrake Ward is already active in the tree, no additional purchase is needed. If it is unlocked but unpurchased, spend one perk point to activate it. The Night of Horrors is also listed with two Skill Points among its rewards, which can cover that cost if your version requires it.[4]
How Mandrake Ward Works After Unlocking It
Mandrake Ward has one exact combat effect: when lethal damage would kill Coen, the passive automatically restores three Health Segments. It requires no input, menu activation, timing window, or consumable. The perk functions as a single automatic revive for that night.[4][6][9]
The limit is tied to the entire night cycle, not to each fight. If Mandrake Ward triggers during a minor ambush, it will not save Coen again against a boss or later elite encounter until the next night begins. Keep that in mind when entering dangerous content with low health: the passive can be consumed before the encounter where it matters most.[6][9][14]

- Limit: one activation per in-game night.
- Reset: the following night cycle.
- Best use: high-damage boss fights, multi-stage encounters, and late-game night routes.
On lower difficulties, Mandrake Ward is still valuable as a recovery buffer. On higher difficulties, it is one of the most consequential Vampirism perks because it can prevent a full loss after a single lethal mistake. It does not replace healing management or defensive play, but its three-segment recovery gives enough health to disengage, heal further, or finish an encounter safely.[6][14]

Refusing the mandrake is the only decision in this route that permanently prevents Mandrake Ward for the playthrough.
