The annoying part is that community reports describe two different failure points: the original Season of Infernal Chaos tracker stalling on Acquire and equip a Chaos Perk, and later reports on newer builds where the quest does not spawn required Memory Fragments. The fix depends on which version of the bug you actually have.

Fix the Chaos Perk step first

If your objective text refuses to advance after you unlock perks, start here. This is the most common and most fixable version of the bug.

• Go to the seasonal interface or the Chaos Perk menu from the quest flow in Kyovashad.
• If a perk is already sitting in a slot, remove it, salvage it, or extract it if your current build gives you that option.
• Equip a Chaos Perk again by hand instead of relying on the quest prompt to auto-complete.
• Close the menu completely and wait a few seconds for the server-side tracker to catch up.
• If nothing changes, log out to character select and load the same seasonal character again.

This works because the quest often is not failing at the reward step; it is failing at the flag that confirms you actually equipped the perk. Re-equipping forces the game to check the condition again. Seasonal hotfixes improved that behavior, but did not remove every edge case, especially on characters created around launch week.

If a normal relog does not fix it, back out further and do a short realm swap. Enter an Eternal Realm character or simply load another character, return to the main menu, then go back to your seasonal character. Players have reported that this hard refresh catches the tracker more reliably than a simple town portal or menu reopen.

If the Kyovashad seasonal board will not advance

Another common stall happens at the board interaction in Kyovashad. If the board does nothing, or the quest marker remains active after you interact with it, treat it like a phasing problem before assuming your character is permanently broken.

• Leave your party before retrying. Shared seasonal progress is one of the easiest ways to get quest states out of sync.
• Waypoint out of Kyovashad, then travel back in and interact again instead of chaining attempts in the same instance.
• If you play with cross-network groups, test one retry with cross-network play disabled in Game Menu → Options → Social. Later fixes specifically targeted cross-play desync behavior.
• Do not stack menu actions. Interact with the board, then wait for the quest text to update before opening the perk screen, inventory, or map.

Do this solo if possible. Even when group play is not the root cause, solo phasing gives you one clean variable. That matters in bugged seasonal content because you want to know whether the objective is failing on your character or on the instance state.

What to do if Memory Fragments do not spawn

There is also a later version of this quest bug where required Memory Fragments do not appear at all. Those reports showed up around newer builds and are not identical to the original Chaos Perk issue, so do not expect the perk fix alone to solve it. If your problem is missing fragments, the practical workaround is to reset the objective phase rather than brute-force the empty room.

• Leave the objective area completely and return to town.
• Log out and back in, or abandon and retake the quest if your current build allows it through the Journal.
• Re-enter the objective zone solo and replay the section from the start.
• If fragments still fail to appear, do a full instance reset again instead of repeating attempts in the same phase.

The reason this helps is simple: fragment bugs usually come from the scene not spawning properly, not from your inventory or damage output. If the room loaded in a bad state, staying inside it rarely fixes anything. A clean re-entry gives the game another chance to generate the missing interactables.

Reports on this specific variant are more build-dependent than the perk bug, so it is worth acknowledging some uncertainty here. If your character is from an older seasonal patch, you may see the Chaos Perk stall. If you are on a newer build under the same quest name, you may be dealing with the fragment-spawn version instead. The shared quest name makes the community advice look inconsistent when the underlying failure is actually different.

Use the reputation bypass if the tracker still refuses to move

If you are tired of resetting the same step, there is a slower workaround that can still save the character: advance Viz-Jaq’taar reputation independently until the game catches the next progression trigger. Community reports around Season of Infernal Chaos point to Rank 3 as the most useful breakpoint.

• Complete Chaos Rifts even if the quest marker is still stuck.
• Collect Infernal Warps along the way.
• Aim for roughly 20 Chaos Rift clears and 50 Infernal Warps to reach the reputation threshold that can force the next quest update.
• If you have Torment unlocked, Helltides are a good place to combine Warp farming with regular progression because Chaos enemies show up there as well.

This is not the elegant solution, but it is useful because it turns a hard stop into a grind you can at least control. In practice, it is the fallback for players who cannot get the perk or board steps to flag correctly but still want access to the next seasonal systems.

Patch status and why some characters are still affected

Seasonal fixes in early October 2025 improved this quest substantially. The first hotfix addressed server-side quest flagging, and a later update targeted cross-play desyncs that were leaving some players in a bad state. Even so, community reports never completely disappeared. That is why the bug can feel random: some cases were fixed by patches, some require a tracker refresh, and some later reports appear to be a separate spawn issue under the same quest name.

If your character started the quest before those fixes, expect more stubborn behavior. Older characters often keep whatever broken flag they originally inherited. How to avoid re-triggering the bug on a fresh attempt

If you have to restart the step or replay the quest, keep the run as clean as possible. The safest approach is boring, but boring is good when the game is already unstable.

• Run the quest solo until it advances to the next stage.
• Equip Chaos Armor first if you get it before the perk objective, since some players report smoother progression when the game recognizes both systems in order.
• Do not bounce between menus immediately after interacting with the seasonal board.
• After equipping a Chaos Perk, stand still for a moment and watch the quest tracker instead of instantly teleporting away.
• If the first retry fails, do a full relog before doing more experimental fixes. That preserves time and keeps the test clean.

Once the quest updates and sends you on to Sand of Chaos, you are usually clear of the worst blocker. From there, the important thing is that seasonal progression opens back up: Chaos Rifts, further Viz-Jaq’taar reputation, and the early ladder toward Infernal Hordes Tier 1 all become available again. If your tracker moves even once after one of these resets, keep playing forward rather than reopening the problem step unless the game forces you back into it.
